A Care Journey is a personal caregiving companion designed to bring order, clarity, and confidence to the lives of family caregivers. It serves as a mobile-responsive command center where caregivers can document care goals, organize provider portals and pharmacy information, maintain emergency preparedness plans, and keep track of the technology and devices that support their loved one's care.
The app is built for family caregivers who are doing their best every single day — people managing appointments, medications, and complex care decisions often without a centralized system to rely on. Rather than scattering critical information across notebooks, browser bookmarks, and sticky notes, A Care Journey gives caregivers a single, private place to park everything that matters.
Privacy is foundational. No patient names are ever stored, and all records are encrypted and visible only to the caregiver who created them. The app pairs with a broader caregiving ecosystem that includes a physical workbook, a digital spreadsheet bundle, and personalized advocacy support.

Dr. Trina L. Coleman, BCPA
Dr. Coleman is a Board Certified Patient Advocate (BCPA) with deep experience supporting family caregivers through some of their most challenging seasons. She created A Care Journey to give caregivers the organized foundation they need to advocate confidently and care for themselves along the way.
